#7 & #8 head nuts.....sleeve nuts

has anyone else had problems with the sleeve nuts stripping their threads? 3 so far for me. This has only been in the last year. I'm talking about the stepped down nut.
SO...as you're torqueing the head down and feel the threads giving up look first at the sleeve nut rather than assuming the stud is pulling out of the head. This could save you a lot of work.
 
I will guess that the current nuts are being made of a lesser grade material or perhaps a threading problem. It is only recently that I've had this problem.
 
Daft question, you are giving them 20ft/lb, not 30? Apologies if obvious.

8 & 9 7 & 8 (3/8" studs) are 30 lbs.ft.
9 & 10 (5/16" studs) are 20 lbs.ft.

If you used new nuts and they failed where did you source them?
