There's no washer under the #2 nut (49) and the washers under the sleeve nuts were later deleted although they are still available #50 so you appear to have one washer missing.

Ive never liked the std small diameter washers on the head bolts. They deform the counterbore and squeeze metal into the bolt hole
I touch a counterboring tool into the bolthole just to clean up the seat, making it flat, and then turn up some thicker stainless washers that are a neat fit in the counterbore.
One of the bolts in the photo has been waisted to make it into a stretch bolt
