5 speed issue....I'm stymied

I've been rebuilding/restoring unit Triumphs for the last 20 or so years,but this one has me stymied.
Back Story: I was riding my 74 engined 750 5speed "Hardtail" to a local event when I heard a horrible "bang" come from the gearbox while shifting from 2nd to 3rd gear. At that point my bike seemed to be sitting in a "false neutral" position. I manipulated the lever and was able to get the bike into 1st,neutral,2nd,and 3rd.....it would not shift into 4th or 5th. I was able to get home ok using what I had. When I got the bike apart I was expecting to see bits and pieces of broken parts laying on the gearbox floor. None were there,and the gearbox oil was clean. I disassembled the entire gearbox and primary to inspect All of the drivetrain parts. Everything measured out ok ,shift forks,camplate,gears,bearings and shafts looked great. So I reassembled the entire shooting match paying very close attention to correct assembly and indexing of the quadrant. Guess what?......I still can't get the bike to shift into 4th or 5th gear!!!!

What do you think I may have missed on my inspection that would cause this to happen? Any help would be appreciated.....Thanks,Skip
 
I haven't had issues with 5-speeds, thankfully.

That also means I'm of no help to you.

Paul I also posted my question on the Britbike Forum and John Healy said that early 5 speed boxes had a problem with the high gear on the layshaft drifting out of position. I'll remove the layshaft and take a closer look. Who would know more about this than John Healy ,Right?
